Below you will find a list of the forms to be filed with the Construtcion and Code Enforcement Office for various regulated activities in the Borough. Certificate of Occupancy Form
This form should be used for rentals of residential apartments or single family homes. Certificate of Occupancy Form - Business
This form should be used for all business rentals. Transfer of Title Application
This form should be used when applying for a Certificate of Occupancy for the change in ownership for a residence. Residential single or two-family. Transfer of Title for Businesses
This form should be used when applying for a Certificate of Occupancy for the change in ownership for a commercial business or residential housing of three units or more. Heating System or Hot Water Heater Permit
This Application is for the certification of a Heating System or Hot Water Heater Installation. To be included with all C/O applications. Carbon Monoxide & Fire Detection Inspection Form
An Application for safety device inspection consistent with the Certificate of Occupancy process. To be included with all C/O applications. Commercial Business Checklist

Application for a Zoning Permit from the Zoning Officer is needed before any fence, shed, sidewalk/driveway, pool, addition or new structure permit will be approved. Fence Permit Form
Application to install a fence. Shed Permit Form
This Form is required for building or having a shed delivered that is smaller than 10' x 10' (100 square feet) on any property in the Borough. If the shed is larger than 100 square feet, a building permit is required. Sidewalk or Driveway Work Permit **
Application for replacement of sidewalk and/or driveways within the Borough.
**Please note if your driveway is on a county road and you are working with any curbs or aprons, a county road opening permit will be need first. County roads in Keyport are as follows: W. Front Street, Broadway, Maple Place, Green Grove Avenue, First Street and Broad Street. Pool Certification Form
